New Patient Assistance
Our specially-trained Care Delegates are knowledgeable, sensitive professionals eager to answer questions and guide you to the specialized care you need.
Nobody likes doing paperwork in a doctor’s office, so we’ve made it easy to download New Patient Information documents. Please complete all of the forms our Care Delegate requests and bring them with you to your appointment. We’ll review them with you in person.
Insurance Information
We participate with Medicaid and most major insurance companies. New patients will meet with a Care Delegate to review and verify insurance information.
- Please bring your insurance card(s) with you. A photocopy will be stored in your records.
- If you’re covered by more than one insurance company, please let us know which is the primary carrier to avoid delays in receiving your benefits.
- If your health coverage changes, please contact us to update your records.
Insurance requirements differ greatly. We strive to make this complex process easier and help you understand what your coverage requires. Please know some insurance companies require that you use a specific laboratory, or obtain pre-authorization for office visits, hospital admission and treatment. These requirements are not in our control.
Co-payments are due at the time of your visit. Visa, MasterCard and Discover are accepted.
